Requirements
All clinical notes must be completed and signed within 72 hours of the encounter. • 48 hours is the standard expectation. • Same-day completion is expected when feasible.
Definition of Completion
A note is considered complete when it is finalized, signed by the rendering provider, and includes all required clinical elements per organizational and regulatory standards.
Compliance & Enforcement
Notes not completed within 72 hours may be subject to non-payment. • Repeated non-compliance may result in further administrative review and corrective action. • Compliance will be monitored regularly through documentation audits and reporting.
Exceptions
Exceptions may be considered under circumstances such as system outages, approved leave, or other extenuating circumstances as determined by leadership. Providers must communicate proactively when barriers arise.
Responsibility
All providers are responsible for adhering to this policy. Leadership will provide support and oversight to ensure compliance.
